We understand that there is an overwhelming amount of information about diabetes care that you must absorb quickly. Here is some of the practical information you might need as well. 

How to Make an Appointment: Office visits are an important part of diabetes care. You should expect to have at least three to four visits per year. To make an appointment, call Joslin Pediatric Services at 617-732-2603.

How to Get a Referral to Joslin: Many insurers require that you follow a procedure to obtain a referral from your pediatrician to receive services at Joslin.  Click here for guidelines (PDF) on how to obtain a referral.

Fact Sheet and Consent Form for Communicating by E-mail or Fax: E-mail and fax are convenient for non-urgent communication with your healthcare team, but these have risks as well.
