Young Adult Transition Care Program for Type 1 and Type 2 Diabetes
Joslin Clinic now offers a Transition Care program for young adults with diabetes who are either newly diagnosed or transitioning from Pediatric to Adult Care.
A Transition Care Coordinator is appointed to these patients and ensures that they are regularly scheduled for appointments and will follow up after any missed appointments.
Patients involved in Joslin's Transition Care Program have their care coordinated by a team of endocrinologists, diabetes nurses, nutritionists, exercise physiologists, and psychologists who specialize in young adults with diabetes.
