Centers for Medicare and Medicaid Services (CMS) Chronic Care Improvement (CCI) Programs

Joslin Diabetes Center’s Disease Management Department is working with McKesson Health Solutions on a Chronic Care Improvement (CCI) pilot program for the Centers for Medicare & Medicaid Services (CMS). This coalition will provide disease management services to Medicare fee-for-service beneficiaries in Mississippi who have complex diabetes and/or congestive heart failure.
