Joslin Diabetes Forum 2012: From Prevention to Treatment

Primary care clinicians oversee the management of most patients who have diabetes or are at risk to develop it. The challenge is how to prevent, diagnose, and optimally treat this complex medical condition in a timely and cost-efficient manner in a primary care practice.

You will learn how to use risk-assessment tools in an efficient, systematic way to identify and intervene in patients at increased risk for developing type 2 diabetes and other cardiometabolic risk factors by initiating timely and appropriate risk-reduction therapies, as well as how to improve the effectiveness of diabetes care for your patients, whether you provide that care within your own practice or through referral and coordination with specialists and other area resources.

This certified live activity has been developed for primary care providers and their clinical and office staff. There is no fee to attend this live activity. However, space is limited, and reservations will be accepted on a first-come, first-served basis. Only clinicians can be admitted. Guests who are not clinicians cannot be accommodated.

Participants will be provided with clinically relevant, evidence-based information.
Upon completion of this activity, participants should be able to:

• Describe methods and office-based systems that can be used to identify people who are at increased risk for developing diabetes and other components of the cardiometabolic syndrome
• Demonstrate the ability to identify people who are at increased risk for the development of prediabetes, diabetes, and other cardiometabolic risk factors through appropriate screening methods
• Describe how to determine individualized glycemic control goals
• Summarize how antidiabetes medications and insulin can be utilized to develop a glycemic control program that balances underlying pathophysiology with patient-specific considerations
• Demonstrate the ability to design a glycemic control program using antidiabetes medications and/or insulin in a manner that balances underlying pathophysiology with patient-specific consideration
• Describe markers of increased risk for development of diabetes-related complications
• Implement an office-based strategy for the prevention, diagnosis, and/or treatment of key complications of diabetes
