Joslin's Web Site Privacy Policy

Information Collection and Use

Joslin Diabetes Center is the sole owner of the information collected on this site. We will not sell, share  or rent this information to others except as  disclosed in this statement. Joslin Diabetes Center collects information from our users at several different points on our Web site.

Contact Form and Feedback Forms

We request information from the user on various contact and feedback forms. On these forms, a user must provide contact information (like name and shipping address) and is asked optional information about their diabetes. This information is used to allow us to answer a user’s question and to send information requested to the user. It also gives us more information about who visits the site so that over time we can improve the content of the site to better serve users .

Donations and Purchasing Online

We use an outside company to collect information to allow individuals to donate to Joslin online or to purchase certain services via the Web. These companies do not retain, share, store or use personally identifiable information for any secondary purpose. When a user makes a donation online, third parties are not allowed to use personally identifiable information except for the purpose of providing these services to Joslin.

Registration

For sections like our discussion boards and professional education, individuals are asked to register and provide certain information as part of the registration process. This information may be used to contact the user about other services on our site, and, in the case of online professional education, to confirm that the individual has registered for certain courses or received credits for certain courses, or to send notification of future programs in their area. Much of the requested information is optional, but certain information is required (such as e-mail address, and for professional education, information to confer CME credits). This individual information is not shared with any third party.

E-Mail Communications

The e-mail functionality of our site does not provide a completely secure means of communication.  E-mail communications that you send to us by using the e-mail links on our site cannot be presumed to be confidential. They may be read by various staff at Joslin in order to best respond to your inquiry.  In addition, your e-mail may be read or accessed by other Internet users before it gets to Joslin.  Joslin cannot guarantee the privacy or security of any e-mail that you send to Joslin.

Log Files

An IP address is a number that is automatically assigned to your computer when you log-on to the Internet. We use IP addresses to analyze trends, administer the site, track user’s movement and gather broad demographic information for aggregate use. IP addresses are not linked to personally identifiable information.

Sharing

We will share aggregated demographic information collected through the Contact and Feedback Forms and other mechanisms with our partners and advertisers. This is not linked to any personal information that can identify any individual person. We use outside companies to to administer some aspects of online giving and ordering online, and a credit card processing company to bill users for goods and services. These companies do not retain, share, store or use personally identifiable information for any secondary purposes.

Mailing Lists and Registration Lists

We offer individuals the opportunity to receive newsletters from Joslin or to be added to a Joslin mailing list for more information. Individuals who opt for this service may periodically receive e-mail updates and other types of postal mailings from us. This list is never shared with outside companies, although we may from time to time provide you with information about a company or its services or ask you to fill out a survey on behalf of a partner if we believe it would be helpful to improve the quality of diabetes care. You may be removed from this mailing list at any time by sending a request to be deleted that includes your name, address, and e-mail address to diabetes@joslin.harvard.edu. To be removed from the professional education mailing list, please e-mail cme@joslin.harvard.edu.

This Web site contains links to other sites. Please be aware that Joslin Diabetes Center is not responsible for the privacy practices of such other sites. We encourage our users to be aware when they leave our site and to read the privacy statements of each and every Web site that collects personally identifiable information. This privacy statement that you are now reading applies solely to information collected by this Web site. Please be aware also that a link on Joslin's site does not mean that Joslin endorses or is in any way responsible for the content or the activities of the linked site.

Security

This Web site takes every precaution to protect our users’ information. When users submit sensitive information via the Web site, that information is protected both online and off-line. When our registration/order form asks users to enter sensitive information (such as credit card number and/or social security number), that information is encrypted and is protected with the best encryption software in the industry. All of our users’ information, not just the sensitive information mentioned above, is restricted in our offices. Only employees who need the information to perform a specific job are granted access to personally identifiable information. ALL employees are kept up-to-date on our security and privacy practices. The servers on which we store personally identifiable information  are kept in a secure environment. Supplementation of Information

In order for this Web site to properly fulfill its obligation to its users, it is necessary for us to supplement the information we receive with information from third party sources. For example, to determine if our customers are qualified to make a specified donation or purchase, we check with their credit card company to ensure they have adequate funds available on their card to make the transaction.

Notification of Changes

If we decide to change our privacy policy, we will post notification that the policy has changed on our Homepage so our users are always aware of what information we collect, how we use it, and under what circumstances, if any, we disclose it. If at any point we decide to use personally identifiable information in a manner different from that stated at the time it was collected, we will notify users by way of an e-mail. Users will have a choice as to whether or not we use their information in this different manner. We will use information in accordance with the privacy policy under which the information was collected.

Children

No part of this Web site is designed or intended for children under age 13. We do not collect personally identifiable information from anyone whom we actually know to be a child under the age of 13.
