Contact Form and Feedback Forms
We request information from the user on various contact and feedback forms. On these forms, a user is asked to provide contact information (like name and email address) and may be asked optional information about their diabetes. This information is used to allow us to answer the user’s question and to send information requested to the user. It also gives us more information about who visits the site so that over time we can improve the content of the site.
Donations and Purchasing Online
We use outside companies to collect information to allow individuals to donate to Joslin online or to purchase certain services online. These companies do not retain, share, store or use personally identifiable information for any secondary purpose. When a user makes a donation online, third parties are not allowed to use personally identifiable information except for the purpose of providing these services to Joslin.
For sections of Joslin’s website that require registration for a Joslin program or service, individuals are asked to register and provide certain information as part of the registration process. This information may be used to contact the user about other services on our site, or to confirm that the individual has registered, or to send notification of future programs or services. Mailing Lists and Registration Lists
We may offer individuals the opportunity to receive newsletters from Joslin or to be added to a Joslin mailing list for more information.
