Joslin Diabetes Center places the highest priority on protecting your privacy and the confidentiality of any information you entrust to us. Below, we tell you what information we collect it and how we use it.

Information Collection and Use

Joslin Diabetes Center is the sole owner of the information collected on this site. We will not sell, share or rent this information to others except as disclosed in this Statement. Joslin collects information from our users at several different points on our website.

Confidentiality

A visitor to Joslin’s site may collect information about Joslin or use our website without disclosing personal, identifying information. We will not obtain such information about you unless you choose to submit it to us. If you supply personal, identifying information (such as name, address, telephone numbers or email address) for various purposes on this site or via other sources, we only use the information for that purpose (that is, we will only use it to provide the service or information you have requested).

Privacy and Confidentiality of Your Health Information

Protecting your privacy and maintaining the confidentiality of your health information is an obligation we take very seriously. Learn more about how we protect your privacy by reading Joslin’s Notice of Privacy Practices.

Contact Form and Feedback Forms

We request information from the user on various contact and feedback forms. On these forms, a user is asked to provide contact information (like name and email address) and may be asked optional information about their diabetes. This information is used to allow us to answer the user’s question and to send information requested to the user. It also gives us more information about who visits the site so that over time we can improve the content of the site.

Donations and Purchasing Online

We use outside companies to collect information to allow individuals to donate to Joslin online or to purchase certain services online. These companies do not retain, share, store or use personally identifiable information for any secondary purpose. When a user makes a donation online, third parties are not allowed to use personally identifiable information except for the purpose of providing these services to Joslin.

Registration

For sections of Joslin’s website that require registration for a Joslin program or service, individuals are asked to register and provide certain information as part of the registration process. This information may be used to contact the user about other services on our site, or to confirm that the individual has registered, or to send notification of future programs or services. Much of the requested information is optional, but certain information may be required (such as an e-mail address). This individual information is not shared with any third party.

Security

Joslin uses security measures that will protect the confidentiality of personal information under our control and appropriately limit access to it. Joslin uses encryption technology, such as Secure Sockets Layer (SSL) to protect your personal information during data transport. SSL protects information you submit via our website, such as ordering information including your name, address and credit card number.

Should you electronically request a referral to a Joslin provider, we will use the information you submit to arrange for care where appropriate. While we use SSL encryption to protect sensitive information online, we also do everything in our power to protect user-information offline. Our software and hardware are configured to provide maximum security.

Cookies

The Joslin website uses cookies to track traffic coming to our site. We do not collect personal data such as name, age, telephone number, email, or mailing address. Cookies will also be used to identify a person who chooses to register on our site. If you choose to register on our site and are given a password, choose to give gifts online, or choose to enroll in our email subscription services, we will identify you with cookies. You will have an opportunity to opt out at any time.

Sharing

We may share aggregated demographic information collected through contact and feedback forms and other mechanisms with our partners. This is not linked to any personal information that can identify any individual person. We use outside companies to administer some aspects of online giving and ordering online, and a credit card processing company for donations or services. These companies do not retain, share, store or use personally identifiable information for any secondary purposes.

Mailing Lists and Registration Lists

We may offer individuals the opportunity to receive newsletters from Joslin or to be added to a Joslin mailing list for more information. Individuals who choose this service may periodically receive e-mail updates and other types of postal mailings from us. This list is never shared with outside companies, although we may from time to time provide you with information about a company or its services or ask you to fill out a survey on behalf of a partner if we believe it would be helpful to improve the quality of diabetes care. You may be removed from this mailing list at any time by sending a request to opt out that includes your name, address, and e-mail address to communications [at] joslin.harvard.edu (communications)diabetes [at] joslin.harvard.edu (@joslin.harvard.edu).

This website contains links to other sites. Please be aware that Joslin Diabetes Center is not responsible for the privacy practices of these other sites. We encourage our users to be aware when they leave our site and to read the privacy statements of each and every site that collects personally identifiable information. This privacy statement that you are now reading applies solely to information collected by Joslin’s website. Supplementation of Information

In order for this website to properly fulfill its obligation to its users, it is necessary for us to supplement the information we receive with information from third party sources. For example, to determine if our customers are qualified to make a specified donation or purchase, we check with their credit card company to ensure they have adequate funds available on their card to make the transaction.

Notification of Changes

If we decide to change our privacy policy, we will post notification that the policy has changed on our website so our users are always aware of what information we collect, how we use it, and under what circumstances, if any, we disclose it. Users will have a choice as to whether or not we use their information in this different manner. We will use information in accordance with the privacy policy under which the information was collected.

Children

No part of this website is designed or intended for children under age 13. We do not collect personally identifiable information from anyone whom we actually know to be a child under the age of 13.
