How to Make an Appointment: Office visits are an important part of diabetes care. You should expect to have at least three to four visits per year.

How to Get a Referral to Joslin: Many insurers require that you follow a procedure to obtain a referral from your pediatrician to receive services at Joslin.  Click here for guidelines (PDF) on how to obtain a referral.

Fact Sheet and Consent Form for Communicating by E-mail or Fax: E-mail and fax are convenient for non-urgent communication with your healthcare team, but these have risks as well.
